Fingerprinting Requirements: ^{must meet ALL requirements}

Applicants for all permits or registration cards must present one of the following types of personal identification at your appointment:

• Valid driver's license
• State-issued identification card
• Current U.S. issued passport

FINGERPRINT CARDS

• Fingerprint cards are $10.00 per fingerprint card.
• Applicant must provide a valid ID.

BROTHEL WORK CARDS

You will need to download and complete the application form and waiver forms and bring these to your scheduled appointment.

Brothel Work Card – Courtesan

•  Applicant must provide a valid ID, a completed application, and a signed referral slip from the employer. A medical clearance must be faxed to NCSO by the Medical Provider.

Brothel Work Card – Employee

• Applicant must provide a valid ID, a completed application and a signed referral slip from
  the employer.

LIQUOR WORK CARDS

Liquor work permits are regulated by Nye County Code 5.04.

1. 1. 1. All applicants are required to complete an application packet which includes a form that allows the Sheriff's Office to inform your employer as to the status of your application.
      2. See Service Charge Schedule for cost of new, renewal or replacement work permit.
      3. Fingerprints will be taken at time of application/renewal for both a state-wide and a national records check.
      4. Applicants are required to have a photograph taken for their work permit card.
      5. For all application types - Initial, Renewal, Change of Employment, Replacement, Additional Work Site Card, the applicant must provide a valid ID and the completed application.

Liquor work permits are issued for a period of five years. Convictions of numerous Driving Under the Influence of Alcohol-related offenses may disqualify you from obtaining a work permit.

Nevada Gaming Control Board LiveScan & Background

• Applicant must provide a valid ID and a completed application provided by the employer.

Nye County School District Employee Prints

• Applicant must provide a valid ID and completed application from the Nye County School
  District.

Nevada Department of Education (OPAL)

• Applicant must provide a valid ID and must complete requirements on the OPAL portal.

Health Care Fingerprinting

• Applicant must provide a valid ID and a completed application.

Felon Registration - Initial and Change of Information

• The convicted person must have a completed Felon Registration Form.  
• A valid ID is preferred but not required.

Sex Offender Registration – Initial

• The convicted person must have a completed Nevada Sex Offender Registration Form.
• A valid ID is preferred but not required.

Sex Offender Registration – Verification/Change of Information

• The convicted person must have a completed Nevada Sex Offender Registration Form.
• A valid ID is preferred but not required.
